Applicants must
:

  • Have reliable transportation and be willing to travel throughout all 15 NWGA counties.
  • Be comfortable visiting consumers' homes, nursing facilities, schools, businesses, and community events.
  • Be proficient with Microsoft Office 365, email, online databases, and electronic documentation.
  • Be capable of managing a full caseload independently.
  • Successfully pass a criminal background check.

Position Summary

NWGA Center for Independent Living is seeking a professional who is dependable, self-directed, and committed to serving individuals with disabilities. This position requires professionalism, accountability, and the ability to manage a demanding workload while representing the organization throughout the community.

Primary Responsibilities
  • Assist individuals transitioning from nursing facilities into the community.
  • Coordinate housing, equipment, benefits, and community resources.
  • Work closely with hospitals, nursing facilities, waiver providers, and community partners.
  • Maintain accurate Well Sky and agency documentation.
  • Advocate for consumers throughout the transition process.
  • Travel extensively throughout Northwest Georgia.
Minimum Qualifications
  • Bachelor's degree or relevant human services experience.
  • Knowledge of community resources and durable medical equipment preferred.
  • Reliable transportation and valid driver's license.
  • Strong documentation and computer skills.
  • Background check required.

  • Maintain confidentiality and professional conduct at all times.
  • Meet deadlines and complete documentation accurately.
  • Communicate effectively with supervisors, coworkers, and community partners.
  • Represent NWGA CIL professionally in the community.
  • Failure to meet performance expectations may result in disciplinary action.
