Social Worker Lmsw, Lcsw, Or Lmhc

Rome Health is seeking a licensed social worker or licensed mental health counselor to join our hospital-based behavioral health team. This role provides compassionate, patient-centered mental health services to individuals and families across the continuum of care.

Position overview:

The clinician in this role will provide individual, group, and family therapy while completing clinical assessments and supporting treatment planning. This position plays a key role in ensuring patient safety, coordinating care, and connecting patients with appropriate resources and services.

Key responsibilities:

• Provide individual, group, and family counseling sessions
• Conduct psychotherapy, intake screenings, and comprehensive clinical assessments
• Complete safety planning and provide treatment recommendations
• Participate in treatment planning and ongoing patient support
• Utilize evidence-based assessments and therapeutic interventions
• Provide case management, referrals, and linkage to community resources as appropriate
• Assess risk, including suicidal and homicidal ideation, and develop appropriate safety plans
• Support patients in identifying strengths, needs, and treatment goals
• Collaborate with families and care teams as clinically indicated

Qualifications & knowledge:

• Knowledge of mental health disorders and evidence-based treatment approaches
• Ability to complete risk assessments and develop effective safety plans
• Familiarity with DSM-5 criteria and substance use disorder (SUD) diagnosis support
• Understanding of professional ethics and standards of care
• Knowledge of OASAS 822 regulations
• Strong case management skills, including knowledge of community resources and referral processes
• Ability to assess family dynamics and incorporate them into treatment planning

Required credentials:

• Current New York state licensure as LMSW, LCSW, or LMHC (required)
