Job Summary
The Evaluation Nurse is responsible for conducting comprehensive assessments of prospective and current residents in an assisted living/memory care community. This role evaluates residents' physical, cognitive, emotional, and functional status to determine eligibility for admission, appropriate level of care, and ongoing service needs. The Evaluation Nurse collaborates with healthcare providers, families, and interdisciplinary staff to support quality resident care and regulatory compliance.
Key Responsibilities
- Conduct pre-admission and ongoing resident assessments.
- Travel to hospitals, nursing homes, rehabs, and private homes for move-in evaluations
- Review medical records, physician orders, medications, and care needs.
- Determine suitability for assisted living placement according to state regulations and community policies.
- Develop and update resident service plans and nursing assessments.
- Coordinate with physicians, hospitals, case managers, hospice agencies, and families during admissions and transitions.
- Identify changes in resident condition and recommend appropriate interventions or higher levels of care when needed.
- Ensure documentation is accurate, complete, and compliant with regulatory standards.
- Participate in care conferences and interdisciplinary team meetings.
- Provide education and guidance to residents, families, and staff regarding care needs.
- Assist with infection control, medication management oversight, and quality assurance initiatives as needed.
- Support census development through timely assessments and admissions coordination.
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
- Experience in assisted living, long-term care, home health, geriatrics, or case management preferred.
- Strong assessment and clinical decision-making skills.
- Knowledge of state assisted living regulations and resident rights.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively.
- Proficient with electronic medical records (EMR) systems.
- Ability to walk, stand, bend, and lift as needed during resident assessments.
- Ability to travel locally for off-site evaluations if required.
- Full-time
- On-call or weekend responsibilities may apply depending on facility needs.
