The Advanced Practice Provider (APP) delivers high-quality, patient-centered healthcare in a same-day access primary care setting. This role includes assessing, diagnosing, and treating patients across the lifespan with acute, chronic, and preventive needs that require non-emergent same-day access. The APP works independently and in collaboration with the care team, aligning clinical practice with organizational values, goals, compliance and performance standards.
ESSENTIAL FUNCTIONS Patient Care Delivery- Provide timely evaluation and management of patients presenting for same-day care (walk-ins, urgent appointments, acute issues).
- Conducts comprehensive history taking, physical examinations, and diagnostic assessments to identify health concerns, including urgent and same-day issues.
- Provides evidence-based care across the lifespan (pediatric, adult, and geriatric population).
- Demonstrate flexibility in adapting to a high-volume, fast paced environment with unpredictable patient flow with varying complexity.
- Develops and implements therapeutic treatment plans within scope of practice, considering patients’ physical, emotional, cultural, and financial factors.
- Orders and interprets appropriate diagnostic tests; initiates timely follow-up care and referrals based on results.
- Prescribes and administers medications, immunizations, treatments, and minor procedures in accordance with licensure, privileges, and clinic protocols.
- Applies sound clinical judgment in managing acute, chronic, and emergent medical conditions, escalating care when appropriate.
- Maintains complete, accurate, and timely documentation of patient encounters in the electronic health record in compliance with organizational and regulatory standards.
- Coordinates care across disciplines, ensuring consistent and effective communication among providers, nurses, and care teams.
- Oversees assigned care team workflows to ensure coordinated and efficient delivery of same-day care.
- Provides education and counseling to patients and families regarding diagnoses, treatment plans, medications, lifestyle modifications, and preventative care.
- Support patient-centered shared decision-making.
- Identifies and connects patients with community resources and support services as appropriate for their health and wellness goals.
- Responds promptly to patient inquiries, refill requests, and follow-up needs, maintaining continuity of care and patient satisfaction.
- Practices in accordance with state and federal laws, professional licensure regulations, and organizational policies.
- Demonstrates commitment to patient privacy and confidentiality in compliance with HIPAA and related regulations.
- Follows established infection control protocols, including standard precautions and blood borne pathogen exposure control plans.
