Job Summary:
Provides expert home hospice nursing care, coordinating patient visits and treatment plans in accordance with physician orders and agency policies. Supervises and evaluates Home Health Aides and LPNs to ensure quality patient care and documentation compliance.
Responsibilities:
- Coordinate and deliver comprehensive hospice nursing services to patients in their home setting.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses to maintain high care standards.
- Document patient assessments, visits, and progress accurately using Home Care Home Base (HCHB) system.
- Travel up to 75 miles from the home clinic to scheduled patient visits, ensuring timely care.
- Participate in office meetings and maintain communication with the Clinical Manager.
- Adhere to infection control, safety protocols, and agency standards during all patient interactions.
- Valid RN license in the state of assignment.
- Minimum of 1 year recent experience in adult home health or hospice care.
- Proficiency with HCHB and Oasis documentation requirements.
- Reliable transportation, valid driver's license, and agency-required liability insurance.
- Ability to travel up to 75 miles and work flexible shifts including weekends and potential on-call.
- Strong organizational skills and compassion for hospice patients.
