Position: Licensed Practical Nurse (LPN)

Nurse – LPN

Licensed Practical Nurse (LPN/LVN) to provide and document skilled nursing care under the supervision of a Registered Nurse. Responsibilities include following care plans and physician orders, monitoring patient condition, documenting activities, and coordinating care. All activities follow confidentiality standards and professional boundaries.

Shift & Location

Full‑time overnight shifts in Roseburg, OR—from 8:00 pm to 5:00 am, Monday‑Thursday. Pay is case‑specific, typically $32–$36 per hour; rates may vary by case, acuity, and schedule.

Qualifications
  • Active, unencumbered LPN license
  • Current CPR certification (hands‑on); assistance available after hire if required
  • TB skin test current within the last 12 months
  • Six months or more of hands‑on nursing experience preferred but not required
  • Reliable transportation
Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Continuously observe and assess patient condition; report changes to supervisor or physician
  • Document all activities, assessments, nursing actions, and care coordination promptly
  • Participate in implementing and updating nursing care plans
  • Take appropriate nursing action based on assessment to achieve expected outcomes
  • Recognize changes in patient needs and intervene to prevent or reduce risk
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
  • Apply infection control measures protecting staff and patients per OSHA standards
  • Educate patients and families on disease process, self‑care, and prevention strategies
  • Maintain knowledge of nursing competencies through ongoing education and skill evaluations
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as an LPN/LVN
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
Additional State‑Specific Requirements
  • South Carolina: one year of pediatrics experience
  • California: one year of experience working under current nursing license
  • Louisiana: one year of experience working as a licensed nurse
  • Continuing education as required by state
Preferences
  • Six months of recent experience as a licensed nurse in a clinical care setting
  • Home‑health experience

Physical Requirements
  • Must speak, write, read, and understand English
  • Must be able to travel and commute to patient sites
  • Must lift 50 pounds and reposition patients without assistance
  • Must perform prolonged walking, standing, bending, kneeling, reaching, twisting, sitting, and climbing stairs
  • Must have visual and hearing acuity and a strong sense of smell and touch
  • Must respond physically and mentally to emergency situations in the home or during transport
Environment
  • Must function in a variety of environments, including exposure to allergens, blood, bodily fluids, and infectious diseases
