Complex Care Solutions (CCS) is seeking a Nurse Practitioner Per Diem to perform home and telehealth assessments across Douglas County, OR. This role supports CMS/HEDIS guidelines, documents thoroughly, and ensures timely follow-up with primary care providers and health plans.
We offer a $3,500 sign-on bonus and a 401K with auto-enrollment at 3%. In‑home visit rates apply; telehealth visits may be available. Flexibility for a remote clinical environment is provided.
