Franklin Woods Center, a Genesis Health Care skilled nursing and rehabilitation facility in Essex, MD, is seeking a compassionate Geriatric Nurse Aide to join our resident-centered team. In this role, you will provide hands-on support with daily living activities, mobility, comfort, and personal care for older adults in short-term rehab and long-term care. You will monitor and report changes in residents' conditions, support restorative and therapy plans, and maintain accurate documentation.
Working closely with nurses and therapists, you'll help ensure dignity, safety, and high-quality clinical care in a collaborative, growth-focused environment.
Responsibilities
- Provide direct geriatric nursing care including bathing, dressing, grooming and toileting assistance
- Take and record vital signs, weights, intake/output, and report changes to licensed nursing staff
- Assist residents with mobility, transfers, positioning and use of assistive devices to prevent falls and pressure injuries
- Support residents during meals, monitor intake, and follow any prescribed dietary needs or swallowing precautions
- Maintain resident comfort, safety, and dignity while following individualized care plans
- Document care provided in accordance with facility and regulatory requirements
- Observe and promptly report physical, cognitive, or behavioral changes in residents
- Assist with restorative and rehabilitation activities as directed by nursing and therapy staff
- Practice infection control, standard precautions, and safe handling techniques at all times
- Collaborate with nurses, therapists, and other team members to support a resident-centered care environment
