Home Health Registered Nurse (RN) – Residential Home Health and Hospice

Location:

Roselle, IL (serving Schaumburg, Arlington Heights, Rolling Meadows, and surrounding areas)

Type:
Full Time, 40 hours per week, 8‑hour shift, ongoing staffing.

• Provide and manage patient care plans as a registered nurse case manager.
• Deliver quality patient care with a minimum of one year of RN experience.
• Maintain RN license in the state of employment.
• Graduate from an approved school of practical nursing.
• Maintain a current driver’s license and be willing to drive approximately 20% of time.
• Consistently meet critical deadlines for charting and documentation.
• Self‑manage time, schedule, and work independently.
• Demonstrate proficiency with tablets, mobile phones, and EMR software.
• Pass a required drug screen and background check (including medical and recreational marijuana).
