About the RoleThe Home Care Assistant is a vital part of our team, providing aid to participants in their homes and contributing to care planning as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the position delivers safe personal‑care services to Welbe Health participants as scheduled and directed.
Key Features- Stable, full‑time schedule; you will be assigned participants to fill a full workday and will be re‑assigned in cases of cancellations or unforeseen circumstances.
- Visit 2–5 patients per day depending on participant needs and care plans, working shorter individual shifts that add up to an 8‑hour day.
- Reimburse travel time and mileage between participants.
- Provide hands‑on personal‑care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and safe lifting during transfers.
- Assist with instrumental ADLs: laundry, light housekeeping, shopping, and light meal preparation.
- Measure and document participants’ vital signs as instructed.
- Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes.
- Carry out home exercise programs as indicated in the care plan developed by the IDT.
- Provide translation services as needed.
- High school diploma or equivalency.
- Current Basic Life Support (BLS) / First Aid Certification required.
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
- Must be registered with Home Care Registry prior to the start date.
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Hourly base rate: $21.00 – $25.22, plus bonus potential. Pay is adjusted based on experience and qualifications.
