Registered Nurse - Home Health; SGV

ABOUT THE JOB:

We have an opening for a PER DIEM (field-based) California-Home Health Registered Nurse (RN) to provide skilled nursing visits to patients in San Gabriel Valley and surrounding areas
. This role requires strong clinical judgment, efficient documentation, and the ability to work independently in the field.

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Complete accurate and timely documentation (within 48 hours compliance)
• Perform medication reconciliation at each visit and report changes.
• Provide patient and family education based on diagnosis and care needs.
• Collaborates with Care Coordinators, and interdisciplinary team.
• Maintain compliance with Joint Commission, CDPH, and agency standards.

• At least one year of nursing experience, including in a Home Health or similar environment is generally required (knowledge in OASIS documentation is a plus)
• Strong clinical, organizational, and communication skills.
• Ability to work independently and manage time efficiently.
• Familiarity with EMR systems (MyUnity experience is a plus).
• Being bilingual is a plus (Spanish, Vietnamese, Chinese).
