Job Overview
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. As an LVN, you will provide direct patient care and support to individuals in various home settings. Your primary responsibility will be to assist in the delivery of quality medical care and ensure the well‑being of our patients.
Duties- Administer medications and treatments as prescribed by physicians
- Monitor patients' vital signs and report any changes or abnormalities
- Assist with medical procedures, including wound care, injections, and medical imaging
- Collaborate with other healthcare professionals to develop and implement patient care plans
- Provide health coaching and education to patients and their families
- Document patient information accurately and maintain medical records
- Ensure compliance with all applicable laws, regulations, and policies
- Strong knowledge of nursing principles and practices
- Experience working with individuals in home care settings
- Ability to perform wound care
- Proficiency in using electronic health record systems
- Valid LVN license in CA
- CPR certification
- Experience in Home Healthcare
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
- Strong attention to detail and organizational skills
