The ideal Candidate:
Must be comfortable working out in the market/community, building relationships and helping foster patient care toward Holy Family.
Travel area to be determined but will be within/near Des Plaines, IL area
Able to read, understand, and interpret patient charts to identify patient needs and support appropriate care planning.
Comfortable discussing potential treatment options and care pathways with patients, families, and healthcare providers.
Communicates clearly, confidently, and effectively with patients, family members, providers, and other stakeholders.
Presents as knowledgeable, polished, and credible when interacting with healthcare professionals and community partners.
Demonstrates an assertive and direct communication style while maintaining professionalism, empathy, and respect.
Highly motivated and results-oriented, with the drive to build relationships, generate activity, and achieve established goals.
Serves as a strong advocate for patients and their families, helping ensure their needs and concerns are effectively communicated throughout the care process.
ResponsibilitiesThe Field Liaison will be responsible for coordinating services and identifying patients through the health system that may qualify for post hospitalization services. The Field Liaison will assist with identifying patients and assist the hospital with referrals and prior authorization for services aligned with facility standards. The Field Liaison develops relationships and serves as an educational resource for referring providers, patients and family members.
The Field Liaison coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Field Liaison is consistent with coordination and assesses the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Field Liaison is Responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
EDUCATION, EXPERIENCE, TRAINING
1. Current and valid state LVN, PT, OT, RRT or RN License preferred
2. Current BLS certificate upon hire and maintain current.
3. Minimum of five (5) years of healthcare experience preferred.
4. Bachelor’s degree in healthcare or closely related field preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $35.50 to $51.00.
The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
