- Deliver physical therapy treatments established by the supervising Physical Therapist.
- Improve patient mobility, strength, balance, coordination, and safety.
- Reinforce home exercise programs and educate patients and caregivers.
- Monitor patient progress and communicate recommendations to the supervising PT.
- Collaborate with nurses, therapists, physicians, and the interdisciplinary care team.
- Complete timely and accurate documentation.
- Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
- Passion for helping patients improve function and independence.
- Excellent communication and teamwork skills.
- Home health experience is welcomed but not required—we'll help you succeed.
- Valid driver's license and reliable transportation.
