Position: Licensed Practical Nurse (LPN)

Home Care Licensed Practical Nurse (LPN)

Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Hours

Full-time – Monday to Friday. There will be a weekend and holiday rotation: approximately one weekend every 4‑6 weeks and 2 holidays per year, depending on staffing levels.

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN, and the general territory is the southeast metro area.

Qualifications
  • Current license as a Licensed Practical Nurse (LPN) in the State of MN. License must be active and in good standing.
  • 2 years of experience as an LPN preferred.
  • Ability to travel within the geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance required.
  • CPR Certification
Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient.
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient’s condition to the RN.
  • Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Complete timely and accurate clinical notes including addressing patient’s progress.
