Home Care Licensed Practical Nurse (LPN)
Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Full-time. Monday – Friday. A weekend and holiday rotation will occur roughly one weekend every 4–6 weeks and two holidays per year, depending on staffing levels.
TerritoryThis position is based in Roseville, MN and serves the southeastern metro area. The LPN works directly in patients’ homes.
Qualifications- Current license as a Licensed Practical Nurse (LPN) in the State of MN; license must be active and in good standing.
- Minimum of 2 years experience as an LPN is preferred.
- Valid driver’s license and reliable transportation; proof of auto insurance is required.
- CPR Certification.
- Participate in ongoing interdisciplinary assessment of the patient.
- Provide services ordered by the physician as indicated in the plan of care.
- Recognize and report changes in the patient’s condition to the RN.
- Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provide counseling and education to the patient and family to meet healthcare related needs.
- Complete timely and accurate clinical notes, including documentation of patient progress.
