What You'll Do:
- Deliver skilled nursing visits throughout the Twin Cities Metro.
- Perform wound care, medication management, and patient assessments.
- Educate patients and caregivers on treatments and disease management.
- Communicate closely with RN Case Managers, physicians, and the interdisciplinary team.
- Complete accurate documentation using our electronic medical record.
- Help patients remain independent and avoid unnecessary hospitalizations.
What We're Looking For:
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Strong communication and critical thinking skills.
- Comfortable working independently in a field-based role.
- Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
- Reliable transportation and valid driver's license.
