A Care Plan Can Look Perfect on Paper. Then You Walk Into the Patient's Home. The medication list says one thing. The pill organizer tells another story. The discharge instructions say the patient should be following a specific diet. Their refrigerator tells you what's actually happening. The treatment plan says the caregiver will help. Then you realize the caregiver has questions of their own.

That's home health nursing. As an LPN Visit Nurse, you'll see something that's difficult to understand from a hospital room or nurses' station:
How your patients are actually managing their health  you'll have the opportunity to help make their care plan work in the real world. See the Patient. See the Environment. See the Whole Picture. Home health gives nurses a different perspective. You're not only asking whether a patient is taking their medication. You can help determine why they aren't. You're not simply providing wound care.

You're seeing whether the patient or caregiver understands what needs to happen between visits. You're not just educating someone before discharge. You're helping reinforce that education after they've returned home. Sometimes the most important thing you discover during a visit isn't the reason you were originally sent there. That's where your nursing instincts matter. Your Workday Happens in the Community You'll travel throughout a territory centered around where you live, providing scheduled skilled nursing visits within approximately a 35-mile radius of your home.

Instead of spending your entire shift inside one facility, you'll move throughout your local community. Drive to a patient. Provide the care they need. Educate the patient or caregiver. Document the visit. Then head to your next home. You'll have independence throughout your day while remaining connected to experienced RN Case Managers, Clinical Managers, physicians, therapists, and interdisciplinary clinicians.

What You'll Help Patients Manage at Home Your visits may include:
Wound care and dressing changes Medication management and education Skilled nursing observations Injections and ordered treatments Chronic disease monitoring Patient and caregiver education Reinforcement of physician orders and discharge instructions Monitoring for changes in condition Identifying potential safety or care concerns in the home Communicating clinical changes to RN Case Managers and physicians Coordinating with therapists and other members of the care team Accurate and timely clinical documentation Your goal isn't simply to complete the visit.

It's to help make sure the patient's care continues working after you leave.
