You'll travel throughout a territory centered around where you live, providing scheduled skilled nursing visits within approximately a 35-mile radius of your home.
Instead of spending your entire shift inside one facility, you'll move throughout your local community.
Drive to a patient.
Provide the care they need.
Educate the patient or caregiver.
Document the visit.
Then head to your next home.
You'll have independence throughout your day while remaining connected to experienced RN Case Managers, Clinical Managers, physicians, therapists, and interdisciplinary clinicians.
What You'll Help Patients Manage at HomeYour visits may include:
- Wound care and dressing changes
- Medication management and education
- Skilled nursing observations
- Injections and ordered treatments
- Chronic disease monitoring
- Patient and caregiver education
- Reinforcement of physician orders and discharge instructions
- Monitoring for changes in condition
- Identifying potential safety or care concerns in the home
- Communicating clinical changes to RN Case Managers and physicians
- Coordinating with therapists and other members of the care team
- Accurate and timely clinical documentation
