Texana Center is seeking a Habilitation Coordinator-PASRR IDD to manage services for individuals with IDD in nursing facilities or community settings. The role includes assessment, care planning, advocacy, monitoring and crisis prevention, as well as Medicaid billable Habilitation Coordination and targeted case management duties.
The position requires a Bachelor's degree in a social or human services field and a Texas Driver License, with at least one year of direct /DD experience.
