Position Summary:We are looking for a Physician Assistant (PA-C) who enjoys working independently while being backed by a supportive clinical team. This is a mission-driven, patient-first opportunity to truly connect with your patients and influence their health outcomes.
Responsibilities:- Conduct primary care visits in assisted living and memory care settings
- Develop care plans, manage medications, and complete orders
- Serve as a key communicator between providers, patients, and staff
- Document encounters and assessments using EMR systems
- Certified and licensed as a PA in Minnesota
- Clinical experience with aging adults strongly preferred
- Effective communicator with great follow-through
- Valid driver’s license and reliable transportation
