Job Title: Registered Nurse (RN) - Case Manager
Location: Roseville, CA 95661
Contract Duration: 3+ Months with strong potential for extension
Schedule: Monday–Friday | 08:00 AM – 04:30 PM (8-hour shifts)
Pay Range: $50 - $60/hr – W2
(Travel Pay available if above 50 miles)
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans aligned with medical necessity.
- Collaborate with physicians, interdisciplinary teams, and external partners to ensure efficient care coordination and discharge planning.
- Monitor patient progress, evaluate outcomes, and adjust care strategies to enhance recovery trajectories.
- Facilitate utilization management while maintaining compliance with regulatory and organizational standards.
- Educate patients and families to empower informed healthcare decisions and promote long-term wellness.
- Active California RN license in good standing.
- Minimum 2 years of clinical nursing experience; prior case management experience preferred.
- Strong knowledge of care coordination, utilization review, and discharge processes.
