Position Overview
Full‑time Clinical Supervisor:
Nursing (Sacramento) – All opportunities require field visits to patients in their homes.
The role combines on‑site field work with office duties: spend 2 days per week in the 21
HHS office handling administrative and team‑management responsibilities, and spend 3 days per week (approximately 12 visits per week) supervising RN case managers and providing direct support.
- Oversaw clinicians in the field who provide care and treatment to patients in their residences.
- Managed daily patient activities: handled incoming calls promptly and followed up as necessary to meet internal and external customer needs.
- Coordinated training and educational programs for field nurses to improve clinical outcomes and competencies.
- Assessed staff learning needs and formulated specific plans and goals for the next evaluation period, performing administrative duties and serving as a role model and resource for staff.
- Acted as the first point of contact for field clinicians to assist in patient‑care escalations.
- Coordinated bi‑weekly case conferences with the clinical team to review patients’ progress and revise care plans as needed.
- Reviewed and evaluated each case by examining services provided, conducting record reviews, confirming impressions, and guiding clinicians toward more effective performance and higher quality home‑care services.
- Reviewed patients’ clinical diagnoses, medications, procedures, and clinical courses.
- Assisted clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care.
Salary range: $155,000 – $170,000 per year, plus an annual bonus of $40,000 paid quarterly. Starting pay commensurate with relevant experience. Qualifications- Minimum of two years’ management or supervisory experience within the past five years in a healthcare setting, preferably in home health.
- Graduate of an accredited nursing school;
Bachelor’s Degree preferred. - Current California Registered Nurse (RN) license.
- Current Basic Life Support (BLS) certification.
- Valid California driver’s license with an acceptable driving record.
- All licenses and certifications must be current at the time of hire and maintained throughout employment.
