• Make the initial evaluation visit to Oasis Start of Care (SOC)
• Regularly re-evaluate the patient’s nursing needs
• Initiate the plan of care and necessary revisions
• Provide services requiring substantial and specialized nursing skill
• Initiate appropriate preventive and rehabilitative nursing procedures
• Prepare clinical and progress notes, coordinate services, and inform the physician and other personnel of changes in the patient’s condition and needs
• Counsel the patient and family to meet nursing and related needs
• Participate in in-service programs, supervise, and teach other nursing personnel

• Licensed Registered Nurse in California
• Credentials in specialty areas, i.e., phlebotomy, IV, ET
• Current BCLS, CPR card
• One (1) year of acute care nursing or skilled nursing experience
• Prior experience in home health preferred; new graduates welcome
• Valid California Driver’s License, current automobile insurance, and reliable automobile

• Ability to communicate effectively in English, both verbally and in writing

• Computer knowledge to work on Electronic Medical Records (EMR)
• Knowledge of current Federal, State, and Agency regulations on patient home healthcare
