Job OverviewPublic Health Nurse I and Public Health Nurse II positions in the Maternal, Child & Adolescent Health program and the Health Care Program for Children in Foster Care. Nurses will travel to client residences and conduct field visits, provide nursing care and case management, and collaborate with other agencies.
Essential Responsibilities- Conduct home and field visits to assess health needs and provide nursing care.
- Offer counseling, instruction, and education on health promotion and disease prevention.
- Perform nursing assessments, chart documentation, and develop nursing plans.
- Coordinate and refer clients for medical, dental, mental, and developmental services.
- Collaborate with social workers, foster parents, schools, and community organizations.
- Oversee psychotropic medication management for foster children.
- Maintain and update Health and Education Passport documentation.
- Participate in emergency preparedness activities and clinics.
Education and Training
- Bachelor’s degree in nursing from an accredited institution.
- Certified Public Health Nurse license issued by the California Board of Registered Nursing.
- Valid California Registered Nurse license.
- Valid driver’s license and reliable vehicle, with proof of insurance.
Experience
No prior experience required; training will be provided.
Skills and Competencies
- Knowledge of public health nursing principles and practices.
- Ability to perform clinical assessments and case management.
- Strong communication, organizational, and computer proficiency.
- Sensitivity to diverse cultural, ethnic, and socioeconomic populations.
- Capacity to work independently and under immediate supervision.
