Compensation: $90,000-$130,000 per year , depending on experience

Schedule: Monday-Friday with On-Call and Weekend Rotation

- Conduct hospice admissions, eligibility evaluations, patient education, informed consents, and initial care planning.
- Provide case management support, including symptom management, care coordination, and clinical oversight as needed.
- Complete routine, PRN, and urgent patient visits to ensure continuity of care.
- Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide holistic, patient-centered care.
- Educate patients and families throughout the hospice journey with compassion and confidence.
- Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, or a strong clinical background in ICU, oncology, palliative care, home health, or other complex care settings.
- Excellent assessment skills, admissions experience, and confident clinical judgment.
- Strong communication and relationship-building skills with patients, families, and interdisciplinary teams.
- Highly organized with the ability to adapt in a fast-paced, field-based environment.
- Reliable transportation and willingness to travel throughout our service area.
