Position OverviewWe are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.
This position offers a consistent Monday–Friday schedule, manageable caseload, and a localized territory in the northeast metro, allowing you to build meaningful relationships with patients and families.
What You’ll Do Patient Care & Case Management- Conduct initial and ongoing patient assessments
- Develop and manage individualized hospice care plans
- Administer medications and provide palliative interventions
- Educate patients and families on symptom management and care needs
- Provide emotional support to patients and families
- Manage and coordinate care for assigned patients
- Collaborate with physicians and interdisciplinary team members
- Participate in care conferences and family meetings
- Advocate for patient needs and ensure access to resources
- Maintain accurate and timely clinical documentation
- Supervise and support hospice aides
- Ensure necessary equipment and supplies are in place
- Participate in on-call rotation (5 days/month)
- Contribute to quality improvement initiatives
- Active RN license in Minnesota
- Graduate of an accredited School of Nursing
- CPR certification
- 1+ year of acute care experience required
- Hospice or home care experience preferred
- Strong assessment and clinical decision‑making skills
- Valid driver’s license and insured vehicle
- Predictable Monday–Friday schedule with limited on-call
- Manageable caseload (10–15 patients)
- Localized territory (5–6 communities in the northeast metro)
