RN Case Manager, Hospice

Full-Time | Monday–Friday | On-Call Rotation (5 Days/Month)

Position Overview

We are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.

This position offers a consistent Monday–Friday schedule, manageable caseload, and a localized territory in the northeast metro, allowing you to build meaningful relationships with patients and families.

What You’ll Do Patient Care & Case Management
  • Conduct initial and ongoing patient assessments
  • Develop and manage individualized hospice care plans
  • Administer medications and provide palliative interventions
  • Educate patients and families on symptom management and care needs
  • Provide emotional support to patients and families
Care Coordination
  • Manage and coordinate care for assigned patients
  • Collaborate with physicians and interdisciplinary team members
  • Participate in care conferences and family meetings
  • Advocate for patient needs and ensure access to resources
Clinical & Operational Excellence
  • Maintain accurate and timely clinical documentation
  • Supervise and support hospice aides
  • Ensure necessary equipment and supplies are in place
  • Participate in on-call rotation (5 days/month)
  • Contribute to quality improvement initiatives
Qualifications
  • Active RN license in Minnesota
  • Graduate of an accredited School of Nursing
  • CPR certification
  • 1+ year of acute care experience required
  • Hospice or home care experience preferred
  • Strong assessment and clinical decision‑making skills
  • Valid driver’s license and insured vehicle
