At Suite Living Hospice Care, we provide compassionate, high-quality hospice services to geriatric patients across the Twin Cities. Our mission is to enhance quality of life by delivering expert, patient-centered care that supports both patients and their families during life's most important moments.

We are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.

This position offers a consistent Monday–Friday schedule, manageable caseload, and a localized territory in the northeast metro, allowing you to build meaningful relationships with patients and families.

• Conduct initial and ongoing patient assessments
• Develop and manage individualized hospice care plans
• Administer medications and provide palliative interventions
• Educate patients and families on symptom management and care needs
• Provide emotional support to patients and families

• Manage and coordinate care for assigned patients
• Collaborate with physicians and interdisciplinary team members
• Participate in care conferences and family meetings
• Advocate for patient needs and ensure access to resources

• Maintain accurate and timely clinical documentation
• Supervise and support hospice aides
• Ensure necessary equipment and supplies are in place
• Participate in on-call rotation (5 days/month)
• Contribute to quality improvement initiatives

• Active RN license in Minnesota
• Graduate of an accredited School of Nursing
• CPR certification
• 1+ year of acute care experience required
• Hospice or home care experience preferred
• Strong assessment and clinical decision‑making skills
• Valid driver’s license and insured vehicle

• Predictable Monday–Friday schedule with limited on-call
• Manageable caseload (10–15 patients)
