Position: Hospice Float RN – Flexible Clinical Role (Admissions, Visits, Case Support)

Job Title: Hospice Resource RN – Admissions, Visits & Case Management (RN)

Pay: $90,000 – $130,000 DOE

Benefits: Full Benefits Package

Schedule: Monday – Friday with on-call and weekend rotation

Location: Greater Twin Cities Metro Area, 21-County Service Area

We are seeking a Hospice Resource RN to support admissions, field visits, and case management coverage across our growing team.

What You’ll Do
  • Admissions: Conduct hospice evaluations, explain services, complete consents, and initiate care plans.
  • Case Management Support: Step in to assist with patient care, symptom management, and coordination.
  • Visit Coverage: Provide routine, PRN, and urgent visits to maintain continuity of care.
  • Care Coordination: Collaborate with physicians, NPs, social workers, aides, and chaplains.
  • Documentation: Maintain accurate EMR charting and ensure compliance with hospice regulations.
What You’ll Bring
  • Active Minnesota RN license.
  • Hospice experience preferred or strong background in ICU, oncology, or palliative care.
  • Confidence in admissions, patient assessments, and family communication.
  • Strong organization skills and ability to adapt in a fast-paced, field-based role.
  • Reliable transportation and willingness to travel throughout the service area.
  • A compassionate, patient-centered mindset.
