Job Title: Hospice Resource RN – Admissions, Visits & Case Management (RN)
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Schedule: Monday – Friday with on-call and weekend rotation
Location: Greater Twin Cities Metro Area, 21-County Service Area

We are seeking a Hospice Resource RN to support admissions, field visits, and case management coverage across our growing team.

- Admissions: Conduct hospice evaluations, explain services, complete consents, and initiate care plans.
- Case Management Support: Step in to assist with patient care, symptom management, and coordination.
- Visit Coverage: Provide routine, PRN, and urgent visits to maintain continuity of care.
- Care Coordination: Collaborate with physicians, NPs, social workers, aides, and chaplains.
- Documentation: Maintain accurate EMR charting and ensure compliance with hospice regulations.
- Active Minnesota RN license.
- Hospice experience preferred or strong background in ICU, oncology, or palliative care.
- Confidence in admissions, patient assessments, and family communication.
- Strong organization skills and ability to adapt in a fast-paced, field-based role.
- Reliable transportation and willingness to travel throughout the service area.
- A compassionate, patient-centered mindset.
