Together, we are Caring. Connecting. Growing. Home Infusion Nurse - Partner with Patients. Practice with PurposeAs a Home Infusion Nurse, you'll provide one on one, skilled clinical care to patients where they feel most comfortable-at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering life changing therapies.
You'll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.
In This Role, You Will:- Learn more about our Home Infusion Pharmacy:
Optum Infusion Pharmacy - Work an average of 20-24 hours per week
- Travel within the Roseville, St. Paul, and Minneapolis area to deliver care in patient homes or designated sites
- Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies
- Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders
- Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration
- Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence
- Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries
- Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans
- Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports
- Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection
- Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
- Assist in staff orientation and training, serving as a clinical educator and resource as requested
- Paid Door-to-Door - drive time and mileage fully compensated
- Mileage reimbursement in alignment with IRS Standard mileage rates

Required Qualifications:- Active and unrestricted Registered Nurse (RN) license in the state of Minnesota
- Current BLS/CPR certification from AHA or Red Cross or willingness to obtain prior to start
- 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU)
- Ability to work independently with strong clinical judgment and effective interdisciplinary collaboration skills
- Valid driver's license, reliable transportation, and ability to cover an 80 mile service radius while working 20-24 hours per week
- Home infusion experience
- Familiarity with Microsoft Office and EMR platforms
- Knowledge of Infusion Nursing Society (INS) standards
- Understanding of HIPAA, OSHA, and home infusion accreditation guidelines
- Solid communication, organization, critical thinking, and teaching skills
