Job Title: Hospice Resource RN | Admissions, Visits & Case Management

Company: Superior Hospice

Compensation: $90,000-$130,000 per year, depending on experience

Benefits: Full Benefits Package

Schedule: Monday-Friday with On-Call and Weekend Rotation

What You'll Do
  • Conduct hospice admissions, eligibility evaluations, patient education, informed consents, and initial care planning.
  • Provide case management support, including symptom management, care coordination, and clinical oversight as needed.
  • Complete routine, PRN, and urgent patient visits to ensure continuity of care.
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide holistic, patient-centered care.
  • Educate patients and families throughout the hospice journey with compassion and confidence.
  • Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
What You'll Bring
  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or a strong clinical background in ICU, oncology, palliative care, home health, or other complex care settings.
  • Excellent assessment skills, admissions experience, and confident clinical judgment.
  • Strong communication and relationship-building skills with patients, families, and interdisciplinary teams.
  • Highly organized with the ability to adapt in a fast-paced, field-based environment.
  • Reliable transportation and willingness to travel throughout our service area.
  • A compassionate, patient-centered approach to care.
