The position will complete and oversee a variety of assignments at the discretion of the Chief of Social Work. This position will be responsible for collecting factual information from clients, families, and community agencies. This individual will coordinate with staff in the development, implementation and evaluation of individual treatment plan. This position will gather information in regards to the specific services available to each specific client and will communicate this to family and treatment team.

This position will also be available to interview, assess, and represent clients in order to meet their specific needs. Screening, developing, monitoring, and coordinating placement plans will be a major responsibility of this position.

The responsibilities include, but are not limited to:

Screens, develops, monitors, and coordinates client placement plans. Ensures that services and placement plans meet the client's needs and expectations. Directs the coordination of activities in such a way as to minimize disruptions of the delivery of care or placement. Maintains client records and prepares reports and correspondence related to the work.
