About the Role:

The Hospice Admissions Registered Nurse (RN) plays a critical role in providing compassionate and comprehensive care to patients and their families during the end-of-life journey. This position is responsible for conducting thorough patient assessments to determine eligibility and appropriateness for hospice services, ensuring a smooth and supportive transition into hospice care. The RN collaborates closely with interdisciplinary teams, patients, and families to develop individualized care plans that honor patient wishes and promote comfort.

This role requires strong clinical judgment, excellent communication skills, and the ability to provide emotional support during a sensitive time. Ultimately, the Hospice Admissions RN ensures that patients receive timely, dignified, and high-quality hospice care aligned with their needs and values.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Minimum of 2 years of clinical nursing experience, preferably in hospice, palliative care, or related fields.
  • Strong knowledge of hospice care principles, end-of-life symptom management, and regulatory requirements.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
  • Ability to work independently and manage multiple admissions efficiently.

Preferred Qualifications:

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience with electronic medical records (EMR) systems and hospice-specific documentation software.
  • Background in community or home health nursing.
  • Training in grief counseling or bereavement support.
  • Familiarity with Medicare and Medicaid hospice regulations and billing procedures.

Responsibilities:

  • Conduct comprehensive patient assessments in various settings including hospitals, nursing homes, and private residences to evaluate hospice eligibility.
  • Collaborate with physicians, social workers, and other healthcare professionals to coordinate admissions and develop personalized care plans.
  • Educate patients and families about hospice services, care options, and what to expect during the hospice journey.
  • Document all assessments, communications, and care plans accurately and timely in compliance with regulatory standards.
  • Provide emotional support and guidance to patients and families, addressing concerns and facilitating a compassionate admission process.

Skills:

The Hospice Admissions RN utilizes clinical assessment skills daily to evaluate patient conditions and determine hospice eligibility accurately. Strong communication skills are essential for educating patients and families about hospice care options and providing emotional support during the admission process. Organizational and time management skills enable the nurse to coordinate multiple admissions efficiently while maintaining detailed and compliant documentation. Collaboration skills are used to work effectively with interdisciplinary teams to develop and implement individualized care plans.

Additionally, proficiency with electronic medical records and hospice-specific software supports accurate record-keeping and regulatory compliance.
