Position Overview

USRC is a leader in the dialysis industry, emphasizing diversity and inclusion at all levels. The Home Therapy Registered Nurse trains patients in home hemodialysis (HD) or peritoneal dialysis (PD). The role involves in‑center training and occasional home visits. The nurse is accountable for providing quality care and delivering superior customer service in all internal and external interactions.

Responsibilities
  • Train patients (and/or care partners) in the practice of self‑care for PD or HD following federal and state regulations governing registered nurse qualifications.
  • Conduct home visits to assess the patient's home environment per policy; initially and at least annually thereafter, and more frequently as needed to improve care.
  • Document all nursing services in the Electronic Medical Record, including training sessions, routine and non‑routine interactions, and phone conversations, ensuring continuity of care.
  • Communicate results of patient assessment, reassessment, and ongoing monitoring to physicians, team members, and other stakeholders.
  • Participate in infection control monitoring, implementation, and recording as requested.
  • Maintain familiarity with emergency equipment and procedures, and regularly review emergency preparedness with patients.
  • Manage clinic and patient supply inventory, placing orders in a timely manner to avoid fees and penalties.
  • Track and analyze program and patient performance, using critical thinking and root cause analysis to drive improvements.
  • Obtain routine and non‑routine laboratory tests as ordered, and communicate critical lab values and urgent patient needs to the responsible physician or physician extender.
  • Complete and document monthly review of patient medication profiles, and administer medications as ordered by the physician.
  • Perform on‑call nursing services for nights and weekends on a rotational basis as needed or assigned.
  • Show flexibility with staffing locations and hours to accommodate patient and USRC home program needs; regular and reliable attendance is required.
  • For Home Hemodialysis only: coordinate home evaluation with technical services to ensure electrical, plumbing, and drainage requirements are met prior to patient acceptance into the home program.
  • Ensure required water sampling is completed per policy and product requirements, monitoring and reporting water sample and culture results.
  • Work Monday through Friday, 8 am to 5 pm; modified schedules are possible based on patient caseload. Competitive on‑call pay applies when placed in on‑call rotation. Home Training Nurses rarely work on holidays.
Qualifications
  • Current RN license in the applicable state, maintained as current and in good standing.
  • 12 months of clinical experience in providing nursing care and at least 3 months of experience in the specific dialysis modality (minimum 6 months in Texas).
  • Previous experience caring for patients on maintenance dialysis preferred.
  • CPR certification required prior to patient care assignment.
  • Demonstrated ability to distinguish all primary colors.
  • Successful completion of the Home Therapy Nurse Orientation program and annual demonstration of skills and competency for the assigned modality.
  • Working knowledge of English, with ability to communicate verbally and in writing.
  • Basic computer skills and proficiency in all USRC clinical applications within 90 days of hire.
  • Compliance with any state practice requirements and additional background clearances as needed.

Compensation and Schedule

Compensation depends on experience, qualifications, and skills. The position offers a comprehensive rewards package, including a 401(k) match, healthcare coverage, and other benefits. Work schedule is Monday‑Friday, 8 am‑5 pm, with flexible modifications. On‑call pay is competitive.
