Position: Home Health Registered Nurse (RN)

Location: Sanzie Healthcare Services - Fulton County

Pay rate: Intake/admissions: $70

Supervisory visits: $50

We are currently hiring Professional Registered Nurses in Fulton County.

Requirements
  • Current and unencumbered license to practice in Georgia as a Registered Nurse
  • BLS certified
  • Certificate showing Negative for TB
  • Valid driver's license
  • Photocopy of Security Card
  • Professional Liability Insurance (e.g., NSO)
  • Current proof of auto insurance
  • Reliable transportation
  • COVID‑19 vaccine/screening
  • Physical fitness

Background check and random drug testing are required.

The Company

Essential Functions
  • Monitor and supervise the care of members whose health status and situation involve complex observations
  • Review referrals from CCSP care coordinators, evaluate member needs during initial and subsequent visits, and assign appropriate staff to provide needed care
  • Develop and revise member care plans, review content during each supervisory visit, and communicate revisions to appropriate staff
  • Supervise personal support aides and licensed practical nurses
  • Complete clinical records, including documenting progress notes and supervisory visit entries
  • Review, sign, and date all clinical record entries made by personal support aides for members with complex observations
  • Co‑sign all documentation of LPN supervisory visits within ten days of each visit and follow up immediately on all concerns raised by the LPN
  • Communicate member care needs with other CCSP providers, members, representatives, caregivers, and care coordinators
  • Make recommendations to CCSP care coordinators regarding member services
  • Conduct face‑to‑face visits in members' homes to re‑evaluate the needs of those with complex observations
  • Arrange orientation and in‑service training of staff
  • Educate members, representatives, and caregivers about members' health status, level of functioning, and healthcare needs and report changes to the member's physician
Supervisory Functions of the RN
  • Meet with the member and, when appropriate, the member's representative and/or caregiver in the member's home to conduct a supervisory visit with the member who has complex observations
  • Conduct an in‑home supervisory visit at least once per year to observe and monitor the performance of personal support aides who provide care to members with complex observations
  • Complete timely, pertinent supervisory notes that include the documentation required by CCSP General Standards
The RN Must Have
  • A current Georgia license to practice nursing under the Georgia Professional Registered Nurses Practice Act
  • At least two years of experience in home health services or a related field (preferably working with older populations)
  • The ability to supervise personal support aides
  • The ability to communicate effectively
  • Working knowledge of the Rules and Regulations for Private Home Care Providers
  • Working knowledge of the policies and procedures of CCSP, SOURCE, ICWP, and the GAPP Medicare Waiver Program
Education / Training

Licensed in the Practice of Nursing. Specialized training is required for all personal care.
