Hospice Field LPN

Blue Summit Hospice and Palliative Care seeks a compassionate Hospice Field LPN to provide evening in-home care for patients in the Roswell/Atlanta area, Wednesday-Sunday. The LPN will deliver bedside nursing, symptom and pain management, medication administration, wound care, and patient assessments while collaborating with RNs, physicians, social workers, and chaplains. This role includes updating care plans, educating families on comfort measures, documenting in EMR, and participating in on-call rotation, all in alignment with our mission of dignity, comfort, and respectful end-of-life care.
