Registered Nurse - Senior Care Coordinator - Full-Time - Day Shift

Role

The Care Coordinator RN Sr. assesses patient needs, coordinates care across the continuum, and supports discharge planning for complex cases during hospitalization.

• Conducts psychosocial and clinical assessments
• Collaborates with physicians and care teams
• Manages discharge planning
• Ensures timely care progression
• Acts as a resource for team members
• Mentors new staff
• Participates in interdisciplinary rounds and patient education
• Documents care activities accurately
• Identifies barriers to discharge
• Facilitates post-acute referrals

• Requires an Associate's or Bachelor's in Nursing
• RN licensure (single state or multi-state)
• BLS certification
• Minimum of 3 years' acute care experience plus 2 years in hospital-based case management

• Excellent communication
• Strong assessment and organizational skills
• Ability to handle complex situations
• Knowledge of healthcare regulations
• Community resources
• Must be self-directed, mature, and collaborative
