Social Worker, Crisis Counselor

Blue Summit Hospice and Palliative Care seeks a compassionate Social Worker for the North Atlanta area. In this field-based role, you'll support hospice and palliative patients and families through psychosocial assessments, counseling, care planning, and resource coordination. As part of an interdisciplinary team, you'll help patients navigate end-of-life decisions, connect them with community services, and provide grief and crisis support. Responsibilities

• Provide psychosocial support and counseling to hospice and palliative care patients and families in the North Atlanta area
• Conduct comprehensive assessments to identify emotional, social, and resource needs
• Develop and update individualized care plans as part of the interdisciplinary care team
• Educate patients and families on hospice philosophy, advance care planning, and available community resources
• Coordinate referrals to community services, financial assistance, and support programs
• Advocate for patient rights, preferences, and end-of-life goals
• Document assessments, interventions, and care plans in accordance with regulatory and agency standards
• Participate in case conferences, team meetings, and quality improvement initiatives
• Provide crisis intervention and grief support as needed
• Collaborate closely with nurses, physicians, chaplains, and aides to ensure holistic, patient-centered care
