Spring River Rehabilitation & Care Center is seeking an experienced and dynamic Director of Nursing (DON) to lead and oversee all nursing operations within our skilled nursing facility. This leadership role is responsible for ensuring the delivery of exceptional resident care, maintaining regulatory compliance, developing clinical staff, and driving quality outcomes across the organization.
As the Director of Nursing, you will serve as the clinical leader of the facility, providing strategic direction, operational oversight, and mentorship to the nursing team. You will collaborate closely with the Administrator, Medical Director, department leaders, residents, and families to ensure a culture of clinical excellence, accountability, and compassionate care.
Key Responsibilities- Direct and oversee all nursing services to ensure the delivery of high‑quality, resident‑centered care
- Ensure compliance with physician orders, resident care plans, facility policies, and applicable state and federal regulations
- Lead, supervise, mentor, and develop nursing staff, including RNs, LPNs, CNAs, and other clinical personnel
- Oversee staffing, scheduling, labor management, and nursing department operations to ensure appropriate resident care coverage
- Monitor resident clinical outcomes and implement corrective action plans as necessary to improve quality measures
- Collaborate with physicians, therapy providers, pharmacy consultants, and interdisciplinary team members to achieve optimal resident outcomes
- Ensure accurate and timely clinical documentation within the electronic medical record (EMR) system
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives and support continuous quality improvement efforts
- Maintain oversight of infection prevention and control programs, resident safety initiatives, and risk management processes
- Ensure facility readiness for state surveys and regulatory inspections
- Maintain comprehensive knowledge of CMS regulations, New Mexico Department of Health requirements, PDPM reimbursement methodology, RAI processes, and survey compliance standards
- Participate in budget planning, financial oversight, and resource allocation for the nursing department
- Conduct employee evaluations, coaching, disciplinary action, and performance management activities
- Foster a culture of accountability, teamwork, engagement, and clinical excellence throughout the nursing department
- Serve as a resource and advocate for residents, families, and staff while promoting exceptional customer service and resident satisfaction
- Graduate of an accredited nursing program with a current, active Registered Nurse (RN) license in the State of New Mexico
- Current CPR or Basic Life Support (BLS) certification required
- Minimum of one (1) to two (2) years of nursing leadership or supervisory experience in a skilled nursing or long‑term care setting required
- Comprehensive knowledge of CMS and federal regulations governing skilled nursing facilities
- Strong understanding of PDPM, RAI/MDS processes, reimbursement systems, infection control, and QAPI programs
- Demonstrated leadership skills with the ability to motivate, mentor, and develop multidisciplinary teams
- Excellent communication, interpersonal, and relationship‑building skills
- Strong analytical, organizational, and problem‑solving abilities
- Ability to manage multiple priorities and thrive in a fast‑paced healthcare environment
