Position: Licensed Practical Nurse opening

Licensed Practical Nurse

Spring River Rehabilitation & Care Center is seeking a dynamic Licensed Practical Nurse (LPN) to provide support and assist residents in performing daily living activities. The LPN will support medical staff in administering care to patients and serve as a compassionate advocate for residents and their families.

Key Responsibilities
  • Provide high‑quality nursing care in accordance with care plans, physician orders, and facility standards.
  • Assess, monitor, and document patients’ conditions and communicate changes to the healthcare team.
  • Administer medications and treatments safely and accurately.
  • Collaborate with physicians, CNAs, and other team members to ensure excellent patient outcomes.
  • Educate patients and families on care plans, medications, and health maintenance.
  • Supervise and support CNAs and other nursing staff in daily care activities.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Participate in quality assurance, infection control, and safety initiatives.
  • Respond to medical emergencies and follow established protocols.
  • Uphold professional standards, patient confidentiality, and regulatory compliance.
Qualifications
  • Graduate of an accredited nursing program with a current, active Licensed Practical Nurse (LPN) license by the State Board of Nursing.
  • CPR Certified.
  • Ability to provide compassionate, person‑centered care to residents with diverse physical and cognitive needs.
  • Good collaboration and teamwork skills.
  • Fast and adaptive problem‑solving abilities.
  • Outstanding communication and interpersonal skills to foster relationships with residents, families, and staff.
  • A passion for providing compassionate, high‑quality care to seniors.
