Infection Preventionist / Wound Care Nurse (Hands-On & Program Oversight)
Spring River Rehabilitation & Care Center – Roswell, NM
Position Summary This role combines hands‑on wound care with oversight of the facility’s Infection Prevention and Control Program
. The Infection Preventionist / Wound Care Nurse is responsible for direct resident care (wound assessments, treatments, and rounds) while also leading infection control surveillance, reporting, QAPI initiatives, and regulatory compliance.
This is a dual clinical and program leadership role requiring strong organization, clinical judgment, and the ability to manage both bedside care and reporting responsibilities.
Key Responsibilities- Perform routine wound rounds and complete comprehensive assessments
- Provide and oversee wound treatments, dressings, and interventions
- Develop, update, and communicate care plans with providers and IDT
- Monitor high‑risk residents and implement skin prevention strategies
- Ensure accurate, timely documentation in the medical record
- Lead the Infection Prevention & Control Program in compliance with CDC, CMS, and state regulations
- Conduct infection surveillance, tracking, and trending
- Complete NHSN reporting and ensure regulatory timelines are met
- Lead QAPI initiatives related to infection control and skin integrity
- Conduct audits, monitor compliance, and implement corrective actions
- Provide staff education on infection control practices and wound prevention
- Current RN or LPN license in New Mexico (required)
- Experience in long‑term care or skilled nursing (preferred)
- Strong wound care experience required (certification preferred)
- Infection Prevention experience or willingness to obtain certification (IPCO or similar)
- Experience with audits, reporting, or QAPI preferred
- Strong organizational and time management skills
- A nurse who is comfortable working on the floor while managing program responsibilities
- Someone who can balance hands‑on care with documentation, audits, and reporting
- A team member who can lead education and drive compliance while remaining clinically engaged
