Job Summary
The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles and practices within the full scope of nursing expertise, utilizing appropriate age and population specific standards. The LPN performs testing, recognizes problems, recommends solutions, and responds to emergent situations, ensuring the health, comfort, and safety of residents.

The LPN contributes to resident assessment, administers medications or treatments as ordered, provides technical support, documents interactions, and maintains effective working relationships with residents, healthcare providers, and the public.

• Provide professional nursing care for residents under RN or physician supervision.
• Perform testing and evaluate data to assess resident status.
• Recognize problems, recommend solutions, and respond in emergent situations.
• Ensure the health, comfort, and safety of residents.
• Contribute to resident assessments and administer medications or treatments as ordered.
• Provide technical support to healthcare professionals.
• Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary.
• Establish and maintain effective working relationships with residents, health care providers, and the public.
• Communicate clearly and maintain confidentiality of sensitive information.
• Counsel residents, family members, or resident advocates on prevention and treatment plans.
• Perform other related duties as assigned.

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
• Associate degree in practical nursing is preferred.
• Completed a pre‑licensure nursing program with supervised clinical experience across the lifespan or at least 400 hours as an LPN at another facility.
• Current, unencumbered LPN license with the State Nursing board and/or multi‑state licensure privileges.
• For positions requiring IV therapy/management skills, must have completed an IV therapy course approved by relevant nursing board.
• Obtain and maintain required department‑specific competencies and certifications.
• Complete required annual continuing education units (CEU) and competency training.
