Franciscan Ministries is seeking an experienced MDS Nurse to organize, complete and evaluate resident assessments and care plans in accordance with state and federal guidelines. You will coordinate MDS, CAAS and care planning for each resident and communicate with the interdisciplinary team to ensure the highest level of functioning.
The role requires a nursing degree, current unrestricted RN or LPN license, and at least 2 years of MDS experience.
