Rapid Re-Housing Case Manager
The Rapid Re-Housing Case Manager is responsible for securing and placing Domestic Violence and Human Trafficking survivors in transitional housing through partnership development with specific cooperating landlords as well as creating subsidized rental housing. Case Manager acts as point person to coordinate intake assessments, service plan creation as well as in-house deliveries of employment skills training, education, and economic assistance, in-person therapeutic counseling, children's services, and legal system advocacy, as well as making referrals for other services.
The Case Manager reports to the Associate Director of Housing and will work closely with A Safe Place staff, community leaders, and centers of influence. This position ensures compliance with Medicaid billing requirements, documentation standards, and regulatory expectations, while maintaining adherence to CARF standards and accreditation guidelines.
Position is located in Round Lake Beach, IL (with travel required - within Lake County Only)
Responsibilities:
- Develop partnerships with cooperating landlords and create rental housing opportunities for clients.
- Educate landlords about specific policies that MUST BE FOLLOWED to keep families safe.
- Manage the related application process, any required updates (i.e. income, household size) and re-certifications to help clients maintain stable housing.
- Perform outreach to community-based organizations, housing resources, and landlord associations to identify new and existing housing opportunities and build strong relationships to better assist clients in accessing and transitioning into offsite housing.
- Coordinate intake assessments and eligibility evaluations to determine client's needs and appropriateness for services.
- Conduct VI-SPDAT Assessments on active homeless clients.
- Create service plans in collaboration with clients to work on goals, such as life skills (e.g. budgeting, savings plan), employment assistance, education assistance, economic assistance, therapy, and referrals internally and to outside agencies.
- Conduct housing inspection and complete inspection documents, per grant requirements, prior to client signing a lease and moving into the unit.
- Ensure documentation in client records is appropriate, accurate, timely and in accordance with agency's policies and funding requirements.
- Provide case management with clients and completing home visits as part of the case management; meeting with clients regularly in person as needed.
- Conduct home visits with clients for housed clients.
- Provide 70% Direct Service to clients on a monthly basis.
- Assist clients in housing search by meeting clients on a weekly basis during their search.
- Complete housing assessment and housing summary with client, and all other necessary documentation that pertains to housing, including, but not limited to, rental assistance packets and intake documents.
- Implement prescribed client assessment tools and comply with reporting requirements including ICJIA prescribed evaluation. Ensure documentation in client records is appropriate, accurate, timely and in accordance with Agency's policies and funding requirements.
- Assistance with Crime Victim Compensation claims, when possible, information and referrals, documentation of services provided, and routine follow-up to ensure that the victim's needs are being addressed across multiple systems.
- Provide information on the Illinois Domestic Violence Act, Human Trafficking, Orders of Protection and stalking laws to enhance client safety and abuser accountability
- Attend monthly Coordinated Entry and Housing Placement Workgroup meetings, and other meetings through the county, as needed.
- Work closely with A Safe Place (ASP) staff to refer clients to additional support services within ASP and outside ASP.
- Collaborate with the organizations that refer clients or provide services for ASP's clients.
- Recruit and develop relationships with community and corporate partners, including providing training.
- Attend and participate in staff meetings and Team Meetings as required.
