Are you passionate about supporting youth and families in need? Join our dedicated team at Bluebonnet Trails Community Services as an Intensive Case Manager (Wraparound Facilitator) and make a lasting impact on the lives of children and families facing mental health challenges.

About the Role

As a vital member of our Youth & Family Services team, you will play a key role in assessing the unique needs of youth and their families. You’ll collaborate with a multidisciplinary team to develop individualized care plans, coordinate services, and facilitate Child & Family Team meetings. Your work ensures that services are provided in a strength-based, family-driven, and culturally competent manner.

Key Responsibilities
  • Conduct comprehensive assessments to identify the strengths and needs of youth and families.
  • Develop and oversee individualized Wraparound Plans in partnership with families and service providers.
  • Coordinate services by connecting families with community-based resources and providers.
  • Facilitate Child & Family Team meetings to ensure collaborative decision-making.
  • Monitor treatment progress and evaluate service effectiveness based on data-driven outcomes.
  • Provide education and consultation to providers on Wraparound principles and best practices.
  • Advocate for youth and families while promoting recovery-oriented, evidence-based interventions

  • Ensure all documentation is accurate and completed in a timely manner.

* Range of salary subject to applicable differentials listed below*

Applicable Differentials
  • Up to $4.00/hour, tenure with BTCS
  • Up to $2.00/hour, previous community center experience in a similar position
  • $1.00/hour, advanced licensure
  • .50/hour, LCDC

**$200 Monthly Service Delivery Bonus**

Credentials
  • Minimum Qualifications:

    A Bachelor’s degree from an accredited college or university with a major or equivalent coursework in a social, behavioral, or human service field and knowledge of mental illness. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.
  • If degree is not in a human services field, a minimum of 24 college credits in a human services field are required.
  • Preferred Qualifications:

    One year of experience in mental health assessment and/or crisis intervention and screening.
  • Master’s degree in psychology, social work or a related human service field.
  • Bilingual preferred.
