This position performs routine (journey-level) eligibility and case management work. Work involves clerical, eligibility determination, and some clinical support for all walk‑in clients and phone‑in clients who are seeking health care information and program services assistance. This position will be responsible for responding to inquiries regarding the County Indigent Healthcare Program (CIHCP) as well as providing screening for additional social services programs including SNAP, TANF, Healthy Texas Women, Medicaid, and other services as needed.
Works under general supervision with moderate latitude for the use of initiative and independent judgment.

- Identifies assets and resources for all public inquiries into services provided within the Program Eligibility and Social Services Division.
- Assesses factors affecting health in a community including access to transportation, access to healthy food, access to CIHCP healthcare program, and other related services.
- Completes assessment during initial appointment to identify needs.
- Collects accurate data for public health information for designing, implementing, evaluating, and improving policies, programs, and services.
- Enters client demographic information to database and software systems with accuracy and completeness.
- Adheres to HIPAA laws as they pertain to PHI and release of client information and data.
- Reports abuse or neglect of children or adults in accordance with related policies and procedures.
- Connects individuals to programs and services within the PESS Division, to internal WCCHD divisions, and to partner programs and agencies.
- Participates in QI activities to support and improve policies, services, and organizational performance.
- Contributes to the determination of priority services and organizational initiatives to align with community health needs, the strategic plan of WCCHD, and continuous improvement within existing programs.
- Participates in internal audits to ensure service delivery accuracy and consistency.
- Responds to client inquiries and actively listens to their needs, questions, and information that will assist with timely and effective processing and/or referrals for services.
- Make appropriate and timely appointments for clients based upon processing times, scheduling needs, and to client request.
- Provides clients with list of required documentation for the CIHCP program and other state and federal service programs.
- Explains external resources by providing a thorough review of the resource, what is needed, who to contact, and additional guidance to assist clients in successfully attaining the resource.
- Interprets for Spanish speaking clients as needed.
- Facilitates translation services for non‑English speaking clients.
- Gathers faxes, mail, and drop‑off applications for CIHCP.
- Conducts follow‑up calls and appointments with clients to monitor progress in attaining services, tracking state and federal service applications, and providing additional support as needed.
- Provides referrals to WCEMS Community Health Paramedic program.
- Works within the community through outreach initiatives with service programs including charity health clinics, food pantries, child and education groups, homeless resource agencies, and various community public health events.
- Assists in collaboration efforts with internal and external audiences through outreach activities, community events, local agencies, work groups, and other social service agencies.
- Provides referrals to behavioral health services for crisis, trauma, and mental health needs.
- May participate on a WCCHD committee or task force.
- In the event of a public health emergency, employees may be called upon to support WCCHD's response in ways that are outside the usual scope of their job responsibilities. This may involve working hours that are outside the employee's usual work hours. Employees will not be required to perform duties that are outside of their competence or professional licensure.

- Minimum of 2 years' experience in social work, public health, or related social services field required.
