This part-time position performs moderate (journey-level) eligibility and case management work under the National Council on Aging (NCOA) through the Benefits Enrollment Center (BEC) model. Work involves clerical, eligibility determination, and general administrative support within the program. This position will be responsible for responding to inquiries regarding SNAP, SSI/SSDI, Medicaid, Medicare Part D and/or Low-Income Subsidy, LIS), and the Medicare Savings Program (MSP).

Works under general supervision with limited latitude for the use of initiative and independent judgment.

This is a grant-funded position with an end date of July 31, 2028. Continuation of employment beyond this date is not guaranteed.

Data Analytics / Assessment
  • Assist with screenings, client outreach, recertifications, appointment support, event staffing, and data entry.
  • Enter client demographic information to database and software systems with accuracy and completeness, including but not limited to Indigent Healthcare System (IHS), State of Texas portals, and the Cumulus platform.
  • Adhere to HIPAA laws as they pertain to PHI and release of client information and data.
  • Report abuse or neglect of children or adults in accordance with related policies and procedures.
Policy Development / Program Planning
  • Connect individuals to programs and services within the PESS Division, to internal WCCHD divisions, and to community partner programs and agencies.
  • Contribute to the determination of priority services and organizational initiatives to align with community health needs.
Community / Cultural Competency
  • Respond to client inquiries and actively listen to their needs, questions, and information that will assist with timely and effective processing and/or referrals for services.
  • Make appropriate and timely appointments for clients based upon processing times, scheduling needs, and per client request.
  • Explain external resources by providing a thorough review of the resource, what is needed, who to contact, and additional guidance to assist client in successfully attaining the resource.
  • Facilitate translation services for non-English speaking clients.
  • Conduct follow-up calls and appointments with clients to monitor progress in attaining services and to provide additional support as needed.
Health Equity / Community Partnership
  • Will participate in a minimum of two (2) outreach activities each quarter including, but not limited to, collaborative meetings, network events, community events, information sessions, and general attendance at local food pantries and charitable clinics.
  • Adhere to Texas’s No Wrong Door framework in which warm hand-offs, short- and long-term support, client advocacy, and follow through are fundamental for the reduction of barriers.
  • Provide referrals to behavioral health services for crisis, trauma, and mental health needs.
Other
  • In the event of a public health emergency, employees may be called upon to support WCCHD’s response in ways that are outside the usual scope of their job responsibilities. This may involve working hours that are outside the employee’s usual work hours. Employees will not be required to perform duties that are outside of their competence or professional licensure.
Qualifications
  • High School diploma required.
  • Minimum of 1 year experience in social work, public health, or related social services field preferred.
  • Bilingual (Spanish/English) preferred.
