Nurse Practitioner - Hospice - PRN

Overview

Called to serve patients in their homes, the Nurse Practitioner delivers personalized, one-on-one care to help patients regain independence and achieve their health goals. In this role, you will provide hospice care through evaluation and assessment of patients’ overall medical conditions, complete face-to-face assessments, and evaluate hospice eligibility.

As The Nurse Practitioner, You Will

• Provide medical care, palliate, and manage terminal illness
• Complete history and physical examinations by evaluating the patient’s medical condition and health history, ordering and interpreting laboratory and diagnostic tests, diagnosing health conditions, and documenting findings
• Develop individualized plan of care, prescribe medications and/or treatments, and obtain consultations
• Complete hospice face-to-face visit requirements
• Review and develop protocols for treatment and consult with the collaborating physician when the patient’s plan of care is outside standardized practice and protocols

• Familiar and comfortable with technology; HCHB experience is a plus

Experience To Deliver On Our Mission

• Licensed Nurse Practitioner with state licensure in the state of operation where the assigned branch is located
• Minimum of one year of Nurse Practitioner experience
• Minimum of six months to one year of hospice or pain and symptom management experience
• Reliable transportation and a valid driver’s license
