Job Summary:
Thera Care Home Health is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. In this role, you will manage a caseload of patients, complete comprehensive assessments, and coordinate care across the interdisciplinary team. You will play a key role in supporting patients throughout their home health episode while promoting independence, safety, and positive clinical outcomes.
Qualifications:
Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing Current Registered Nurse (RN) license in the state of Texas Minimum 2 years of RN clinical experience; home health experience preferred
Strong knowledge of home health regulations, OASIS documentation, and care planning
Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred
Ability to manage a patient caseload and work independently in a field-based environment
Experience coordinating care with interdisciplinary teams (PT, OT, ST, MSW, HHA)
Strong clinical assessment, documentation, and critical thinking skills
Excellent communication and interpersonal skills with patients, families, and physicians
Reliable transportation with valid driver’s license and auto insurance
Proficiency with EMR systems (Home Care Home Base preferred)
Essential Functions:
Manage and coordinate all aspects of home health patient care and case management
Perform Start of Care (SOC), Recertification, and Resumption of Care assessments
Develop, implement, and update individualized plans of care based on patient needs
Coordinate care with physicians and interdisciplinary team members to ensure continuity of care Supervise and provide clinical direction to LVNs and Home Health Aides Lead and participate in interdisciplinary case conferences (IDG/IDT)
Monitor patient progress and adjust care plans to achieve desired outcomes
Provide patient and caregiver education on disease management, medications, and home safety
Complete accurate and timely clinical documentation in compliance with regulatory standards
Coordinate discharge planning and ensure smooth transitions of care Monitor visit utilization, productivity, and patient outcomes
Maintain compliance with agency policies, procedures, and state and federal regulations
