Position OverviewThe Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate, implement, and update the nursing care plan.
- Takes appropriate nursing action based on assessment to achieve expected outcomes.
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk.
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Provide care utilizing infection control measures that protect staff and patient per OSHA standards.
- Educate patients and families regarding disease process, self‑care techniques, prevention strategies, and nursing needs.
- Maintain knowledge of competencies related to the nursing profession through educational programs and continued education units.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- Must have reliable transportation.
- TB skin test current within last 12 months.
- Compact licenses transferred to state of residence within 90 days.
- CPR certification with hands‑on component (Aveanna can assist after hire if necessary).
- South Carolina – one year of pediatrics experience.
- California – one year of experience working under current nursing license.
- Louisiana – one year of experience working as a licensed nurse.
- Continuing education as required by state.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Must be able to reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting, and climbing stairs.
- Must have visual and hearing acuity.
- Must be able to respond physically and mentally to emergency situations in the home or during transport.
- Must function in various environments with exposure to allergens, blood, bodily fluids, and infectious diseases.
Other duties may be assigned. Employees must comply with all health‑related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.
